Head and neck cancers, involving sites from the nasopharynx to the subglottic larynx, are frequently devastating cancers that afflict patients around the world. These cancers are frequently locally advanced prior to detection, and require multimodality therapy that is associated with high morbidity. As this book addresses this difficult disease, it accomplishes three main goals.

First, it introduces the etiology and subclasses of squamous cell carcinomas of the head and neck (SCCHNs), and how these factors affect prognosis. Although habitual exposures to tobacco, alcohol, and other agents have historically been the main causes of SCCHN, a rising proportion of oropharynx cancers arise from transforming human papillomavirus (HPV) infection. These two broad classes of SCCHN have significant differences in disease profile and response to treatment, as we discuss.

Second, it summarizes the current state of understanding of the genetic, epigenetic and protein expression changes associated with the various classes of SCCHN. In the past decade, disease pathogenesis of SCCHN has been appreciated to involve deregulation of multiple tumor pathways, including the receptor tyrosine kinases (RTKs) EGFR, ERBB2, ERBB3, c-MET, and IGF1R; transforming growth factor β (TGFβ); Notch; cytoplasmic signaling proteins including PTEN, PI3K, JAK/STAT, and Wnt-responsive β-catenin; mutation control systems, including p53 and the DNA damage repair (DDR) machinery; and hypoxic response. The specific understanding of the action of these proteins in SCCHN is presented here.

Finally, this book defines potential therapeutic targets for improved management of the disease in the future, discussing prospects for improved prediction of prognosis.

Barbara Burtness, M.D. is Associate Director for Clinical Research at the Fox Chase Cancer Center. She is also a co-leader of the Keystone Program in Head and Neck Cancer. As Chair of the Head and Neck Committee in the Eastern Cooperative Oncology Group and a member of the NCI Head and Neck Steering Committee, she helps shape the national agenda for research on all stages of head and neck cancer.

Erica Golemis, Ph.D. is the Deputy Chief Scientific Officer at the Fox Chase Cancer Center. She is also the Co-Leader of Developmental Therapeutics and Co-Leader of the Head and Neck Cancer Keystone at Fox Chase. Her research seeks to define the changes in cell signaling that occur as tumors spread by metastasis and develop resistance to drugs, with the ultimate goal of inhibiting these processes.

This volume provides an integrated account of our current understanding of the functions of D-type cyclins during development and tumorigenesis, with special emphasis on the kinase-independent functions of these proteins. The volume will provide a thorough review of the latest discoveries on the new functions and interacting partners of mammalian cyclin Ds crucial to explain their oncogenic and differentiation properties in different cellular contexts. The volume begins with a historical perspective of how D-type cyclins were first discovered and eventually cloned from cancer tissues, followed by an account on the canonical functions of cyclin Ds during the G1-S transition of the cell cycle. Several chapters will be devoted to review the functions of D-type cyclins as transcriptional regulators and the mechanisms through which these novel functions could impact the tumorigenic process. Also discussed is emerging evidence that points to a role of D-type cyclins, particularly cyclin D1, as a cytoplasmic regulator of various cellular functions. This property, in human cells at least, is traceable to certain splice isoforms with novel oncogenic implications. Finally, a chapter is devoted to recent efforts to revise the canonical view of the “retinoblastoma pathway” to incorporate new evidence that suggests that cyclin D1’s role in G1 is to singly-phosphorylate the retinoblastoma protein (pRb) for discrimination of target protein interactions. This work represents a significant departure from the view of cyclin D1 as a negative regulator of pRb and may have critical implications for understanding the function of antineoplastic agents that target the cyclin D1-associated kinases.

This book provides up-to-date information on all aspects of ductal carcinoma in situ of the breast, including epidemiology, imaging, pathologic and biologic features, interventional diagnostics, nonpalpable lesion localization, and treatment. Surgical procedures are described in detail, covering breast conservation techniques, conservative mastectomies, breast reconstruction options, and axillary surgery. Guidance is provided on how to ensure adequacy of surgical excision and avoid local recurrence when performing breast conservation surgery and how to minimize morbidity from axillary surgery. The role and techniques of partial and whole breast irradiation are described, and the use of adjuvant systemic therapy options, including endocrine therapy and chemotherapy, is explained. A concluding chapter addresses the issue of recurrence and its current management. Carlo Mariotti is Head of the Functional Unit of Breast Surgery at Ancona University Hospital and Professor of Breast Anatomy and Surgery at the Polytechnic University of the Marche Region, Ancona, Italy. Professor Mariotti graduated from Bologna University Medical School in 1979 and gained his Habilitation in Surgery in the same year. He specialized in General Surgery and Thoracic Surgery, and developed his career on the university surgery ward in Ancona. He mastered in Senologic Studies in Paris in 1984. He has since taught general surgery, clinical surgery, pathological surgery, and reconstructive surgery. He took up his present position in 2003, and since 2005 he has also been director of the National School for Breast Surgery. Professor Mariotti is a member of the Italian Surgical Society and the Italian Association of Hospital Surgeons. He has contributed to numerous research papers and scientific articles and has acted as a moderator and speaker at both national and international congresses.
