Left Atrial Appendage Closure: Mechanical Approaches to Stroke Prevention in Atrial Fibrillation

Left Atrial Appendage Closure: Mechanical Approaches to Stroke Prevention in Atrial Fibrillation

Percutaneous left atrial appendage (LAA) closure is an emerging technology for thromboembolic prevention in patients with atrial fibrillation (AF). The first human implantation of an LAA device occurred in 2001, and since then four devices have received CE mark approval. These devices are being widely used in Europe for LAA closure in patients who are poor candidates for long-term oral anticoagulation. In the US, the WATCHMAN device (Boston Scientific) is anticipated to receive FDA approval imminently for AF patients who are warfarin-eligible. This approval is projected to significantly expand the indications for LAA closures worldwide. Thus, the volume of procedures is anticipated to escalate. This book discusses the epidemiology of AF as a cause of stroke; the use of LAA closure in the reduction of thromboembolism with AF; early surgical approaches and novel surgical devices for LAA closure; and current percutaneous approaches and devices available for LAA closure. The emphasis of this book is on percutaneous technical approaches and contemporary trial results on the leading devices (PLAATO, WATCHMAN, Amplatzer Cardiac Plug, and LARIAT). It also reviews unapproved devices in development, in both clinical and pre-clinical phases.

Cardiovascular Hemodynamics: An Introductory Guide

Cardiovascular Hemodynamics: An Introductory Guide

The second edition of this key resource provides a broad and fundamental overview of basic cardiovascular (CV) hemodynamic principles with a focus on clinical assessment of CV physiology. Extensively updated, the book includes new coverage on noninvasive hemodynamic assessment and the effects of selected interventions on CV hemodynamics. It provides an introduction to the basic concepts such as preload, afterload, myocardial contractility, and cardiac output. Subsequent chapters examine the effects of interventions such as vasodilators, beta blockers, pressor agents, inotropes, and different forms of invasive circulatory support. The book also focuses on various methods of hemodynamic evaluation including echocardiography, CT/MRI, noninvasive hemodynamic assessment, and cardiac catheterization. The book concludes with a discussion of proper diagnosis, evaluation, and management of patients using hemodynamic data on a variety of specific disease states.
An invaluable contribution to the Contemporary Cardiology Seriesthe Second Edition of Cardiovascular Hemodynamics: An Introductory Guide is an essential resource for physicians, residents, fellows, medical students, and researchers in cardiology, emergency medicine, critical care, and internal medicine.

Congenital Heart Disease and Adolescence (Congenital Heart Disease in Adolescents and Adults)

Congenital Heart Disease and Adolescence (Congenital Heart Disease in Adolescents and Adults)

This book provides medical care givers with detailed information on those aspects of adolescence that are of significance in the setting of congenital heart disease (CHD), from anatomic and physiologic changes to behavioral issues. In addition, it explains how care should be organized in order to ensure that the needs of adolescents with CHD are fully met. Both theoretical and practical aspects of the switch from a pediatric to an adult health perspective in CHD patients are outlined in detail, drawing attention to the importance of a structured transition plan and other best practices. As the survival of children with complex CHD improves further, the number of adolescents with CHD will continue to grow. Awareness of the challenges that these patients face is essential if they are to be appropriately prepared to assume adult roles and functioning. Readers will find Congenital Heart Disease and Adolescence to be an excellent source of relevant knowledge and guidance. It has been written for a broad audience, bearing in mind that care in adolescents is an interdisciplinary task involving close collaboration among physicians, specialists, nurses, patients, and relatives.

Pregnancy and Congenital Heart Disease (Congenital Heart Disease in Adolescents and Adults)

Pregnancy and Congenital Heart Disease (Congenital Heart Disease in Adolescents and Adults)

This book describes the risks facing patients with congenital heart disease who wish to become pregnant and the ways in which these patients can best be followed and treated during pregnancy. In addition, the organization of care around the delivery is discussed in detail. The first section provides clear advice on pre-pregnancy risk estimation, counseling of patients, medication use, and inheritance. The specific risks associated with congenital heart defects of differing severity are then explained, with guidance on monitoring and management. The coverage includes simple lesions such as atrial and ventricular septal defects, conditions associated with moderate risk, including tetralogy of Fallot and coarctation, and complex disease such as a Fontan or Mustard repair. The final section is devoted to delivery and considers the mode of delivery, anesthetic use, and postpartum care. Readers will find much information that is underreported in the literature, and the book goes well beyond the European Society of Cardiology guidelines, for example, by considering medical conditions not defined as high risk and addressing the organization of care thoroughly.

Pulmonary Hypertension in Adult Congenital Heart Disease (Congenital Heart Disease in Adolescents and Adults)

Pulmonary Hypertension in Adult Congenital Heart Disease (Congenital Heart Disease in Adolescents and Adults)

This book is intended as a comprehensive, practically oriented reference on pulmonary hypertension within the context of adult congenital heart disease (ACHD). After an introductory chapter on pathophysiology, the various types of pulmonary hypertension that may be encountered in ACHD are discussed, highlighting the specifics observed within different patient categories. The diagnostic approach is then addressed in detail, and the last section of the book is devoted to management options, from conservative approaches to interventional treatment and the concept of treat and repair. Management in specific patient subjects, such as pregnant women, Fontan patients, and Down syndrome patients with Eisenmenger syndrome, is fully discussed, and guidance is also provided on palliative care.
Pulmonary arterial hypertension related to congenital heart disease (PAH-CHD), despite significant similarities in lung pathohysiology, differs significantly from other types of PAH in terms of mechanism of onset, natural history and management. Mistakes and pitfalls in the management of patients with PAH-CHD are often related to a lack of knowledge or expertise in this condition. Pulmonary Hypertension in Adult Congenital Heart Disease will be a valuable resource and learning tool for all who care for patients with ACHD, both in tertiary practice and general cardiology.
