We live in an era of personalized medicine and the knowledge about pathophysiology of diffuse gastric cancer has had many advances. Thus, the role of this work is to clarify what is new from diagnosis to treatment of this disease in order to treat patients in the most tailored manner as possible.

Almost all phase III trials in gastric cancer have been performed without taking in consideration histologic subtypes, i.e. they have disregarded the differences between diffuse gastric cancer and general gastric cancer. However, the clinical practice reveals that diffuse gastric cancer is a completely distinct disease, with an aggressive course and generally worse prognosis. The loss of cohesion between tumor cells due to the loss of E-cadherin synthesis is the critical point on the oncogenesis of diffuse gastric cancer and is at the root of its marked heredity. This book intends to give special attention to Diffuse Gastric Cancer as a particular oncological entity, differentiating it from general gastric cancer, exploring and discussing all its peculiarities, and addressing the basic aspects (pathology and genetics) along with the most recent therapeutic alternatives for this condition.

This book highlights minimum standards relating to the management of different conditions in the practice of Obstetrics and Gynaecology. The editors explore clinical governance issues, common causes of as well as ways to avoid litigation.

The UK is experiencing a dramatic increase in medico-legal claims. The 4 main reasons for litigation are: accountability, the need for an explanation, concern with standards of care and compensation. However the decision to take legal action is determined not only by the original injury, but failure to provide information, an explanation and an apology. Insensitive handling of an injury and poor communication after the original incident increases the risk of litigation and erodes the patient-doctor relationship. Doctors almost never deliberately cause harm to patients, however increasingly claims are being defended successfully.

This book is invaluable to clinicians and lawyers alike and raises awareness of how to avoid facing clinical negligence claims in our day to day practice.

There are many changes in the skin, hair, and nails during and after pregnancy. Patients are certainly aware that those changes are occurring, but few physicians and even fewer patients know how to predict the course of those changes. Pigmentary changes occur, but are they permanent? Can they be prevented or reversed? Hair may become thicker during pregnancy, only to fall out in the postpartum period. When will hair stop falling out? Will it grow back? What hormonal changes are occurring to produce these effects on the skin, hair, and nails? Will those hormonal changes affect other organs? Some cutaneous manifestations of pregnancy are common and recognized by most physicians. Abdominal striae, for example, are easily identified, but physicians have many questions about them. Why do they develop? Can they be prevented or treated? Other cutaneous manifestations of pregnancy are less common and recognized by few. For example, pruritic urticarial papules and plaques of pregnancy is a debilitating condition that is treatable once diagnosed. Even more important than the treatment, the knowledge imparted to a worried mother that her child will be fine and that the condition is se- limited is priceless; but the condition must first be recognized by the patient’s physician. All of these questions and conditions are addressed in this extraordinary book. Pigm- tary disorders are addressed in detail in Chap. 2.
