Essentials of Hypertension: The 120/80 paradigm 1st ed. 2018 Edition

The main purpose of this book is to select and present the most essential information about hypertension. It aims to select all the more relevant data to guide the attitudes to prevent, diagnose, and treat hypertension.

Hypertension accounts for more than 50% of deaths from stroke and ischemic heart disease worldwide. New blood pressure (BP) diagnostic thresholds for hypertension are coming out, and they will likely be set at 120-130/80 mmHg. As a consequence, nearly two thirds of adults in the world will be at greater risk of a CV event. Prevention and control of high BP will become the main focus for reducing the burden of CV disease, requiring a changing of cultural beliefs in some way similar to what happened in the last century with smoking.

Strategies for prevention of the rising of BP with age, and the BP reduction in individuals already with high levels, are more complex than those related to smoking control. These strategies involve solid evidence to be implemented in populations. The extensive scientific literature dealing with hypertension and BP regulation is among the top dedicated to a single disease. The chapters and contents follow the clinical reasoning pathways. The characterization of the risks of high blood pressure is presented in the first chapter, discussing the evidence that led to changes in diagnostic thresholds and to the recommendations for maintaining BP within these limits in populations. Reasons for BP rising with age will follow, identifying the causes that must be fought to preventing the incidence of hypertension. Diagnosis of hypertension deserves a special chapter. The final chapter presents the fundamentals to select drug and non-drug therapies indicated in the prevention and controlling of high blood pressure.

Full professor of Cardiology of the School of Medicine, Federal University of Rio Grande do Sul, Porto Alegre, Brazil. Training abroad (post-doctoral and sabbatical): UNC-Chapel Hill (1986-87), Johns Hopkins (1996-97) and Harvard/Brigham (2013). Fellow of the American Heart Association. Current positions: Chief of Cardiology, Hospital de Clínicas de Porto Alegre, Researcher CNPq 1A (Brazilian Agency of Research). Editorial Board: Hypertension (past), Journal of Hypertension and others; reviewer for more than 30 international journals (mostly devoted to hypertension and CV disease). Dr Fuchs has a prolific scientific production, mainly on the field of cardiology and hypertension, with 11 textbooks published in English and Portuguese and more than 300 full articles. These contributions have more than 3300 citations and a H index of 30 in ISI and more than 7300 citations and H index of 45 in Google Scholar.

Angela Maas, MD PhD, has been a clinical cardiologist since 1988 and has developed an interest in heart disease in women. She is currently one of the most influential female doctors in Dutch healthcare.

Noel Bairey Merz, MD, received her medical degree from Harvard Medical School, after which she was chief medical resident at the University of California, San Francisco. Bairey Merz also completed residencies in both clinical and nuclear cardiology at Cedars-Sinai Medical Center in Los Angeles, where she is currently the director of the Women’s Heart Center and also of the Preventive and Rehabilitative Cardiac Center. Her research interests include women and heart disease, mental stress and heart disease, the role of exercise and stress management in reversing disease, and the role of nutrition in heart disease. She has been at the forefront of the National Institutes of Health (NIH)-sponsored WISE (Women’s Ischemic Syndrome Evaluation) initiative and currently leads many other health and research initiatives. She is globally recognized as one of the field’s leading experts on women’s heart disease.

She received both her MD in Medicine in Surgery and PhD in Biotechnologies and Biosciences and Cardiology Specialization at G.D’Annunzio University. She worked as a Research Fellow at the University of Texas Medical School and then as a Research Scientist at Texas Heart Institute Center for Cardiovascular Biology and Atherosclerosis Research, both in Houston Texas. She also held the position of Research Scientist at the Institute of Molecular Cardiology, University of Louisville in Louisville, Kentucky. Author of over ninety papers and fifteen book chapters, she is an ex-officio nucleus member of the European Society of Cardiology Working Group in Cellular Biology of the Heart, and Councilor of the Italian Society of Cardiovascular Research (SIRC). Her research interests include atherosclerosis, stem cells, and regenerative medicine.

Phospholipases generate lipid signaling molecules through their hydrolytic action on phospholipids and are known to regulate function of a variety of cells under normal and diseased conditions.
